Approved By Column missing, Approval Date and Approved Version not working

I ran through all the install instruction got no errors and 3 of the 4 columns Showed up.  When I added the columns to a document library and create a document to display them only the Current Version will update and only on check out.  It does not update when a document is published or approved.  Any suggestions?  I am trying this on my test server before deploying to production and this is so far a show stopper.

Problem Solved I had a custom column the already exists with the same name as the column being installed with the solution.  I reset the dev environment and renamed the column before install and everything worked like a champ.
